It looks like Sansa was given the necklace in the 1st episode by the fat dude she saved. Then the same fat dude helps her escape in the 2nd episode. The grandmother appears to have taken something from the necklace of Sansa during the 2nd episode. GRANDMA GUILTY! There's a key player you haven't touched on but who will be revealed next week. Since you seem to want to be spoiled, Olenna was the one who placed the poison jewel (from Sansa's necklace) in the cup.
